P.I.E.C.E.S.™ Learning and Development Programs

Authentic involvement of the person and care partner in his/her health and health care is central in the P.I.E.C.E.S.™ TEAM approach. Meaningful support requires the development of effective caring relationships that honour the uniqueness of the person.  The P.I.E.C.E.S. approach enhances quality of life by recognizing the persons areas of strength and need related to Physical health, Intellectual abilities, Emotional wellbeing, Capabilities, Environment (social and physical) and his or her Social self (cultural, spiritual, Life Story).

P.I.E.C.E.S.™ is Dynamic. 

P.I.E.C.E.S.™ Learning and Development Programs are highly interactive, learner focused and integrate opportunities for practical application of the P.I.E.C.E.S.™ TEAM approach. The P.I.E.C.E.S.™ template is easily integrated into practice to help shape collaborative TEAM conversations, promote continuous individual and TEAM learning, and realize improved outcomes.

P.I.E.C.E.S.™ provides a practical yet comprehensive TEAM solution finding template for assessment and supportive care strategies that promotes an interprofessional approach. P.I.E.C.E.S.™ complements and can be integrated with other best practices related to assessment and capacity enhancement in the care of the complex older person.

P.I.E.C.E.S.™ Enhances Capacity. 

P.I.E.C.E.S.™ enhances capacity at the individual, TEAM, organization and system levels supporting the health and health care of the older person living with progressive or persistent health challenges related to neurocognitive and/or mental health needs and associated behavioural changes.

P.I.E.C.E.S.™ Provides a Common Language and a Common Approach. 

The P.I.E.C.E.S.™ approach provides a shared understanding of the often multiple underlying causes influencing the persons and care partners’ quality of life, behavioural expression and associated risks. A common set of values, a common language for communicating across the system and a common yet comprehensive approach to collaborative care are embedded within the model, enabling a shared accountability for person and care partner-directed health and heath care across the system of care. P.I.E.C.E.S.™ provides a mechanism for collaborative conversations between practitioners, teams, and leaders, as well as between organizations.
